Steps to obtain the CLEARANCE CARD needed to participate in Athletics
Every student participating in a fall sport MUST obtain a clearance card from the Athletic Office prior to participating in any tryout or practice.  To obtain a clearance card, each student must:
1.  Complete an Athletic Release and Physical Form (click here to download).  Page 1 must be fully complete with parent/guardian signature and insurance information. Page 2 of the form must be completed by a Physician (MD/DO), Physician’s Assistant (PA), or Nurse Practitioner (NP) licensed in the State of California. This completed form must be turned into the Athletic Office (Summer Hours: Monday-Friday, 8:00 a.m.-1:00 p.m.).
2.  Complete the student health forms AND the athletic forms online.  Each student must login with their Student ID and password to access their forms. The Student Health Forms have 3 pages of information that need to be completed. When you submit on the 3rd page, after checking “yes” for participating in athletics, you will be redirected automatically to the Athletic Forms. There are 7 pages of information that need to be completed before submitting. If it doesn’t allow you to submit, please be sure to go back and review ALL the pages prior to ensure you answered all the required questions. Sometimes a question missed on the first Athletic Forms page can be the problem. All forms must be completed and submitted electronically before a Clearance Card can be issued.
3.  Each family must register, attend and sign-in at parent/guardian education meeting for Drug and Alcohol Awareness.  An attendance list will be provided to the Athletics Office to verify. If a parent/guardian has not completed this requirement yet, a temporary Clearance Card can be issued that allows the student to tryout/practice. Student may NOT compete in games/competitions/scrimmages/extra curricular activities through the school until this requirement is met. (More information on this in next section)
Once these forms are completed and attendance at the Drug and Alcohol Awareness meeting is verified, the Athletic Office may print a clearance card for each student. The student or parent/guardian must come into the Athletic Office to have one printed out for them. Clearance cards must be given to the coach in order to participate in tryouts.
Transfer students (not incoming 9th graders) must meet with the Athletic Director to complete CIF Transfer Forms prior to obtaining a clearance card.  Please call the Athletics Office at (858) 523-4022 to schedule an appointment.
Clearance Cards are available now for pickup.
The Athletic Office hours are Monday-Friday, 7:00 a.m. – 3:00 p.m.
Cathedral Catholic will require at least one parent/guardian to attend the annual meeting to explore and discuss the perceptions and realities of alcohol and drugs that are pertinent to Cathedral Catholic High School students. In partnership with the San Diego Police Department, Cathedral Catholic administrators and counselors will discuss ongoing concerns about alcohol abuse, increased access to marijuana, the vaping culture, prescription drug abuse and more.

Students whose parent/guardian do not attend the meeting will not be eligible to attend or participate in any extracurricular activities, including athletics, at Cathedral Catholic High School.  Students whose parent/guardian are unable to attend a meeting until August 8th will be eligible to attend tryouts/practices (with a temporary clearance card) but will NOT be allowed to participate in any scrimmages/games/competitions until the requirement is met.
