I hereby authorize the Directors of the CCHS Sports Camps to act for the legal guardians in their best judgement in any emergency requiring medical attention, and hereby waive and release the Cathedral Catholic High School from any and all liabilities for any injuries while attending the CCHS Sports Camps.
