Make checks payable to: Cathedral Catholic High School 5555 Del Mar Heights Road Student Questionnaire - Due by January 8, 2010 Applicants should complete in their own handwriting, sign and return. If required, an additional sheet of paper, stapled, will be accepted. High School Placement Testing - January 23, 2010 Only Freshmen applicants are required to take a standardized High School Placement Test (HSPT). By mailing in your application before January 8th, your son/daughter will be pre-registered for the test. Students who take the exam at CCHS are given priority consideration for admission. Student Recommendations - Due by February 12, 2010 One recommendation form is to be completed by both of the applicant’s English and Math teachers providing information on the student’s academic progress, study habits, conduct, activities, and interpersonal skills. If required an additional copy of the form for each teacher can be made. Recommendation forms are to be mailed from the current school. CCHS will not accept recommendations submitted by the parents or students. Request for Transcripts – Due by February 12, 2010 Deliver form directly to the student’s current school. A transcript of grades and copies of standardized testing for the past two grades, as well as CSIR immunization records, will be needed for the application process.
